wow thats crazy! It almost looks like the casing was too large and as zillacon said too much of a charge.

I've seen some examples of "factory remanufactured" ammo that was way out of spec, too. I remember seeing a set of photos of reman'd ammo that had some of the bullets seated way too far into the brass. Or could have been a squib load that lodged a bullet in the barrel, then the next shot had nowhere to go but KA-BOOM!

My money is on a squib. Either way, looks like a reload gone bad.
